Filler Asymmetry Correction in London: Why One Side Settles Differently

Facial balancing before and after, front view showing a more balanced face, Sheridan France London

It is usually day three or day four. You are in the bathroom, the light is unflattering, and you have tilted your head twice to check. One side looks fuller than the other. Or higher. Or the lip on the left has a slightly different curve to the right. You take a photograph, then another one on the front camera, then compare it to a picture from six months ago, and by now your heart is going.

This is one of the most common reasons patients contact a clinic in the first week after treatment, and it is worth saying clearly before anything else: at day four, you are not looking at your result. You are looking at a face that is still settling, still holding fluid unevenly, and still adjusting to a product that has not yet fully integrated with your own tissue. Most early asymmetry after dermal filler resolves without any intervention at all. A smaller number needs a small, considered adjustment. A very small number needs product reduced.

What follows is how to tell the difference, what a proper review appointment involves, and why the practitioners who place less at the first pass tend to have the simplest job at review.

Why one side settles differently in the first two weeks

Swelling is the single biggest reason a fresh filler result looks uneven. It is also the least symmetrical thing your face does.

Swelling does not resolve at the same rate on both sides

Lymphatic drainage differs from side to side in almost everyone. If you sleep predominantly on one side, if you have had dental work, sinus problems or a previous injury, or if one side simply drains a little more sluggishly, the fluid you are holding after treatment will not clear evenly. Hyaluronic acid is hydrophilic, meaning it attracts water, so the first few days after treatment often show more volume than the product itself accounts for. That extra water is rarely distributed fairly.

Product has not yet integrated

Hyaluronic acid fillers need time to settle into the tissue plane they were placed in. In the first week the gel can sit slightly more discretely than it will at week four, when it has softened, spread marginally and taken on the movement of the surrounding tissue. Lips are the clearest example. A lip at day five and the same lip at week three can look like two different treatments, and one side often catches up to the other later than you would like.

Bruising distorts what you are seeing

Even a small bruise that has not yet surfaced can create a firmness or a shadow that reads as asymmetry in the mirror. Bruising, swelling and tenderness are recognised, common effects of any injectable treatment, and they rarely appear symmetrically because they depend on which tiny vessels were disturbed on each pass.

Your face was not symmetrical before treatment

This is the part that surprises people. Nobody has a symmetrical face. The difference is that before treatment you were not studying yourself under a bathroom downlight three times a day.

Bone and soft tissue differ from side to side

Most faces have one cheekbone that projects slightly more, one eyebrow that sits marginally higher, a jaw that is a touch fuller on one side, and a nose that deviates a degree or two from the midline. Dental history, orthodontic work, habitual chewing side and long-term sleep position all contribute. A photograph mirrored down the centre line looks unmistakably strange, and that is true of everyone.

Volume can make an existing difference more visible

Adding volume to a face amplifies its architecture, including the parts of that architecture that were already unequal. If your left mid-face sat 1mm lower than the right, restoring projection to both sides can make that 1mm read more clearly, not less. A skilled injector anticipates this and adjusts placement and quantity accordingly, but it cannot always be fully neutralised in one session, particularly when the underlying difference is skeletal.

Attention bias is real

Once you have looked for asymmetry, you cannot unsee it. Patients frequently bring in a pre-treatment photograph at review, and in a meaningful proportion of cases the same difference is visible in the original image. That is not a criticism of the patient. It is simply how attention works after a treatment you were nervous about in the first place.

When uneven filler results genuinely need addressing

Reassurance has limits, and there are situations where waiting is the wrong answer.

Contact the clinic straight away if you notice

  • Blanching (a white or pale patch) of the skin during or shortly after treatment
  • Dusky, mottled or bruise-like discolouration spreading beyond the injection point
  • Pain that is disproportionate, worsening, or starts after an initially comfortable recovery
  • Any change in vision, or pain around the eye
  • Increasing redness, heat, tenderness or fever in the days or weeks afterwards

These are not asymmetry. They are potential signs of vascular compromise or infection, which are rare but recognised complications of dermal filler, and they require prompt medical assessment rather than a wait-and-see approach. Any clinic worth attending will want to hear from you the same day.

Otherwise, the two-week rule applies

For simple unevenness with no other symptoms, the sensible course is to wait until around fourteen days before making a judgement. By then swelling has largely resolved, bruising has cleared, and the product has integrated enough to represent your actual result. Most clinics, including ours, schedule a review at approximately two weeks for this reason. It protects you from chasing a moving target.

What a filler review appointment involves at our Fitzrovia clinic

A review is a proper clinical appointment, not a quick glance in the doorway. If asymmetry is what brought you in, it deserves twenty minutes of structured assessment.

Assessment at rest and in movement

Faces behave differently when still and when animated. A cheek that looks balanced at rest can lift unequally when you smile, because the muscles of facial expression are not equally strong on both sides. Assessment is done sitting upright in good, even lighting, at rest, smiling, speaking and with the head in several positions. Bathroom mirrors and phone cameras both distort, particularly at close range.

Palpation

Some of the most useful information is felt rather than seen. Gentle palpation tells the practitioner whether a raised area is product, oedema or firm early fibrous tissue, and whether the gel sits in the intended plane. That distinction changes the plan entirely.

Photographic comparison

Standardised pre-treatment photographs taken at the same distance, angle and lighting are compared with how you look now. This is where pre-existing asymmetry is usually identified, and it is often the single most reassuring part of the appointment.

The plan

There are four honest options, and a good practitioner will tell you which one applies. Continue to wait. Add a very small amount to the lesser side. Mould or massage a discrete area where appropriate. Or reduce product with hyaluronidase in the minority of cases where placement, quantity or migration means adding more would compound the problem.

What conservative correction actually looks like

Adding a little to the lesser side is usually the first move

Where a genuine imbalance remains at two weeks and the existing product is well placed, the usual approach is a small quantity added to the lighter side, often a fraction of a millilitre. The aim is to bring the two sides into agreement, not to match them millimetre for millimetre, which is neither achievable nor natural-looking.

When dissolving is the right answer

Hyaluronidase, an enzyme that breaks down hyaluronic acid, is used when the issue is too much product, product in the wrong plane, migration beyond the intended border, or a nodule that will not settle. It is a valuable tool used judiciously, and it has its own considerations, including the possibility of taking down a little of your own hyaluronic acid alongside the filler and a small risk of allergic reaction. If you are researching this route, our guidance on dissolving dermal filler in London covers it in more detail.

Why under-filling at the first pass makes review straightforward

This is the quiet advantage of a conservative injector. If a practitioner places a modest amount and reviews at two weeks, any adjustment is additive, low-risk and quick. If a practitioner places the maximum at the first sitting, the only remaining option when something is uneven is to remove, which is a longer, less predictable process. Building in stages is slower and occasionally requires a second appointment, and it is far kinder to the face. It is the approach we take across non-surgical facial rejuvenation and dermal filler treatment at the clinic.

If your filler was placed somewhere else

Many patients who come to us for a review had their treatment elsewhere and feel awkward about it. There is no need. Bring whatever information you have: the product name, the volume used, the date, and any photographs from before. If you have none of that, we work from clinical assessment. The first appointment is a consultation, not an automatic treatment, and in a fair number of cases the recommendation is to allow more time before deciding anything.

Why Patients Choose Sheridan France

Sheridan France holds an MSc, is a Registered Nurse and an Independent Nurse Prescriber, and has spent over twenty years in facial aesthetics. That prescribing qualification matters in this context, because it means complications and corrective work can be managed directly rather than referred on.

Her training was completed with Plastic Surgery Partners on Harley Street, The Consulting Suite on Portland Place, and with the late Dr Frederic Brandt in Coral Gables, a practitioner known internationally for restraint and precision rather than volume. That philosophy runs through the work here: build gradually, review properly, and favour the result that nobody can name. The clinic holds a 4.9 out of 5 rating from more than 339 Google reviews, which we take as a reflection of how patients are looked after when something needs a second look rather than only when everything goes to plan.

You can read more about the clinic and Sheridan’s background at sheridanfrance.co.uk. If you are worried about an uneven result, whether it was our work or someone else’s, request a consultation at our Fitzrovia clinic on 0207 631 5001 and we will assess it properly.

Frequently Asked Questions

Why is my filler uneven after four days?

Most commonly because swelling is resolving at different rates on each side, because a bruise has not yet fully surfaced, or because the product has not finished integrating with the tissue. Hyaluronic acid also attracts water in the early days, which exaggerates volume unevenly. Assessment at around two weeks gives a far more accurate picture.

How long should I wait before asking for a filler review appointment?

Around fourteen days for straightforward unevenness. If you have blanching, spreading discolouration, disproportionate or worsening pain, any visual change, or signs of infection, contact the clinic immediately rather than waiting.

Can uneven filler be corrected without dissolving it?

Often, yes. Where existing product is well placed and the imbalance is one of quantity, a small addition to the lesser side is usually sufficient. Dissolving with hyaluronidase is reserved for excess product, poor placement, migration or persistent nodules.

Will my face ever be perfectly symmetrical?

No face is perfectly symmetrical, and treatment does not aim for that. The goal is balance and proportion that reads as natural in movement, not a mirrored image. Chasing exact symmetry tends to produce results that look artificial.

Do you review filler that was placed at another clinic?

Yes. Bring any details you have about the product and volume used, along with earlier photographs if possible. The appointment begins as a consultation, and in many cases the advice is to allow more settling time before any further treatment is considered.

Request your consultation

If one side of your face is not behaving the way you expected, a calm clinical opinion is worth more than another week of mirror-checking. Every face is different, and any plan begins with a personalised consultation. Results vary from patient to patient depending on anatomy, lifestyle and aftercare, and all injectable treatments can carry a small risk of bruising, swelling or tenderness at the injection site. Dermal filler treatments are not suitable during pregnancy or breastfeeding.
