Tear Trough Fillers in London: Softening Tired Under-Eyes Without Looking Done

You catch your reflection in the back of the lift on the way up to a meeting and the first thing you notice is the shadow under your eyes. Not the meeting prep. Not the new lipstick. The shadow. Colleagues have started asking if you slept well; you did. Your children, if you have them, ask why you always look so tired. You do not feel tired. You simply look it, and you have started to feel betrayed by the face you see in photographs.

If that resonates, you are in good company. The under-eye area is the single most common concern raised at consultations in our Fitzrovia clinic, and it is also the area where patients arrive most worried about what treatment might do. You have seen the photographs on social media: bluish swellings, puffy ridges, cheeks that look filled rather than restored. The fear of looking worse, not better, is entirely rational. This article walks you through what a tear trough filler in London actually involves, who is and is not a suitable candidate, and how a conservative, assessment-led approach produces results your colleagues notice without ever being able to name.

Why Under-Eyes Look Tired in the First Place

The hollow under-eye look is rarely about sleep. It is anatomy. The tear trough is the small ligamentous groove that runs from the inner corner of the eye diagonally down towards the cheek, and as the face matures the supporting fat pads above and below this groove begin to shift. The cheek fat descends slightly. The orbital fat above can pseudo-herniate forward. The skin overlying the area, the thinnest skin on the entire face, becomes more translucent. The result is a shadow that reads, to anyone looking at you, as fatigue.

Some people are born with a deep tear trough. Others develop it in their thirties and forties. It is not a sign that you have done anything wrong; it is, in most cases, simple inherited bone and soft-tissue architecture combined with the gradual changes of ageing.

The Three Things That Create the Shadow

When we assess the under-eye area in consultation, we are looking at three contributing factors and working out the balance between them for your individual face:

  • Volume loss in the mid-face and along the tear trough itself, which deepens the groove.
  • Skin quality, particularly translucency, pigmentation and fine crepiness that allows the underlying vasculature to show through.
  • True under-eye fat prominence (eye bags), which is a different problem entirely and is not usually appropriate for filler.

Understanding which of these three is driving your appearance is the single most important step in the entire process. Get this wrong and you get a poor result. Get it right and the outcome can be remarkably refined.

What a Tear Trough Filler Actually Is

Tear trough treatment uses a small volume of hyaluronic acid dermal filler placed precisely along the bony groove beneath the eye, generally on the deep plane against the orbital rim. The aim is not to inflate the area. The aim is to softly support the hollow so light no longer falls into it as a shadow.

The volumes involved are tiny. We are often working in fractions of a millilitre per side. A heavy hand here is the single most common reason patients arrive at our clinic after treatment elsewhere unhappy with how they look. The under-eye skin is too thin and too mobile to forgive overcorrection, which is why our approach is conservative by design.

The Product Matters

Not all hyaluronic acid fillers are suitable for the tear trough. Products used here need to be soft, low in water-attraction, and behave predictably in thin-skinned tissue. We tend to favour fillers specifically formulated for delicate facial zones, and we will discuss which product is appropriate for you during your consultation rather than applying a single product to every patient.

The Treatment Itself

Treatment takes around 30 to 45 minutes, including time to mark, anaesthetise and place the product. Many practitioners, including ourselves, use a blunt-tipped cannula rather than a sharp needle for this area, which can reduce the risk of bruising and is generally more comfortable. You will feel pressure rather than pain. Most patients return to work the same day, with minor swelling that settles over the following days.

Who Is a Good Candidate, and Who Is Not

This is the part of the consultation that takes the longest, because honesty here protects you from a result you will not be happy with. Tear trough filler is a precise tool, not a universal solution.

You Are Likely a Good Candidate If

  • Your under-eye concern is primarily a hollow or shadow rather than a puffy fat pad.
  • Your skin quality in the area is reasonable, without significant crepiness or pigmentation.
  • The mid-cheek beneath the hollow is reasonably supported (or can be supported with a small amount of cheek filler first).
  • You are realistic about what filler can and cannot achieve, and you want refinement rather than transformation.

You May Not Be a Good Candidate If

  • Your concern is true eye-bag prominence (herniated orbital fat), where filler can make the area look worse, not better. Lower-blepharoplasty surgery is the appropriate route for some patients in this category, and we will say so plainly.
  • You have very thin or pigmented under-eye skin where filler is likely to cast a bluish hue (the Tyndall effect).
  • You have a history of significant fluid retention or chronic puffiness, as hyaluronic acid attracts water.
  • You are pregnant or breastfeeding.

Around one in five patients who come to us asking about tear trough filler leave the consultation with a different plan, sometimes cheek support first, sometimes skin treatments, sometimes a recommendation to consult an oculoplastic surgeon. That is not a failure of the consultation. That is the consultation working as it should.

What Natural Under-Eye Rejuvenation Looks Like

The result you should be aiming for is the one your friends cannot quite place. They might say you look well, that you have been on holiday, that something about you seems brighter. They should not say you have had your under-eyes done. That, in our view, is the standard.

Achieving it relies on three principles we apply to every tear trough treatment at the clinic:

  1. Treat the cause, not just the symptom. If the real problem is a descended cheek, filling the trough alone will not solve it. Mid-face support, placed conservatively, often does more for the under-eye than placing product directly under the eye.
  2. Less, then review. We routinely undertreat at the first appointment and review at two to four weeks. You can always add a touch more product. You cannot subtract overnight.
  3. Respect the anatomy. The infraorbital artery, the angular vessels, and the delicate lymphatic drainage of the lower eyelid all sit in this zone. Slow, deep, cannula-based placement against bone is, in our hands, the safest technique for this area.

How Long Results Typically Last

Most patients find tear trough results last between nine and 18 months, though the under-eye area is notoriously variable. Some patients metabolise product more slowly here than elsewhere on the face, which is another reason we treat conservatively. We would rather you returned for a small top-up than carried excess product for two years.

Why Patients Choose Sheridan France

Sheridan France has practised facial aesthetics for over 20 years and holds an MSc alongside her qualifications as a Registered Nurse and Independent Nurse Prescriber, a combination that remains unusually strong in the aesthetics field. She trained at Plastic Surgery Partners on Harley Street and at The Consulting Suite on Portland Place, and completed advanced training with the late Dr Frederic Brandt in Coral Gables, Florida, a practitioner whose influence on natural-looking injectable work is felt throughout the global aesthetics community.

The philosophy of the clinic is straightforward: results should be the kind colleagues notice without being able to name. The clinic holds a 4.9/5 Google rating from more than 339 patient reviews, which we mention only because the consistency of those reviews speaks to the consistency of the work. Patients who come to us after treatment elsewhere often arrive seeking refinement rather than more product, and that calibration is the heart of what we do.

What to Expect at Your Consultation

The first appointment is an unhurried 45-minute conversation. We look at your face in motion as well as at rest, in different lighting conditions, and against photographs you might want to share of how you used to look. We discuss your medical history, any previous treatments, your lifestyle, and what you are hoping to feel as much as look. Only then do we discuss whether treatment is appropriate, and if so, what the staged plan should look like.

For some patients we will recommend proceeding the same day. For others we will recommend a cooling-off period, a different starting point on the face, or no treatment at all. The recommendation depends on your face, not on the diary.

Frequently Asked Questions

Will tear trough filler get rid of dark circles?

Tear trough filler can reduce the appearance of shadow caused by a hollow, which is often perceived as a dark circle. It does not address pigmentation in the skin itself, which is a different concern requiring skin-led treatment. During consultation we assess which type of darkness you have, since the two often coexist and each needs a different approach.

Does the treatment hurt?

Most patients describe the sensation as pressure rather than pain. We use topical anaesthetic where appropriate, and most modern tear trough fillers contain a small amount of local anaesthetic built into the gel. Using a blunt cannula rather than a needle generally reduces both discomfort and the likelihood of bruising. All injectable treatments can carry a small risk of bruising, swelling or tenderness at the injection site.

How soon will I look normal again after treatment?

Most patients return to work the same day or the following morning. Some mild swelling and the occasional small bruise can be expected; both are usually easy to conceal with makeup after the first 24 hours. The final settled result is generally visible at two to four weeks, once any residual swelling has resolved and the product has integrated with the surrounding tissue.

What happens if I do not like the result?

Hyaluronic acid fillers are reversible with an enzyme called hyaluronidase, which can dissolve the product if required. This is one of the reasons we favour hyaluronic acid in the tear trough area. It is also the reason we treat conservatively in the first instance: a less-is-more first appointment, followed by review, almost always produces a better outcome than aiming for a complete result on day one.

Can men have tear trough filler too?

Yes. The under-eye consultation is one of the most common appointments male patients book, often because they have been told repeatedly at work that they look tired. The treatment principles are the same, though we tend to work to slightly different aesthetic landmarks to preserve a male facial structure.
